Retatrutide 20 mgGIP / GLP-1 / glucagon triple agonist

Retatrutide 20 mg is a 39-amino-acid synthetic peptide developed by Eli Lilly (code LY3437943). It is the first “triple agonist”: a single molecule that activates three hormone receptors that control appetite and energy use, those for GIP, GLP-1 and glucagon. It is still in phase 3 clinical trials and is not approved as a medicine in any country.

Retatrutide: studied effects

• Strong reduction in appetite and slower stomach emptying (GLP-1 effect)
• Large weight loss: 24.2% of body weight on average after 48 weeks at the highest dose in the phase 2 trial
• Higher energy expenditure and fat burning (glucagon effect)
• Better blood sugar control and lower HbA1c in people with type 2 diabetes
• Large reduction in liver fat in fatty liver studies

What the research says

In the phase 2 trial published in the New England Journal of Medicine in 2023, participants on the highest dose lost 24.2% of their body weight on average over 48 weeks. The most common side effects were digestive (nausea, diarrhea, vomiting), mostly while the dose was being increased, along with a faster heart rate. The phase 3 program (TRIUMPH) is under way.

Preparation and storage

The product ships as a lyophilized powder, to be reconstituted with bacteriostatic water. Keep the powder in the freezer or the fridge, away from light. Once reconstituted, keep the vial at 2 to 8 °C and use it within about 4 weeks.
